Care Access & Case Management

We partner with clients and empower them to increase their self-sufficiency

AIDS Action staff members work individually with clients to address barriers to good health, such as lack of insurance, homelessness or unstable housing, unmet mental health and substance use needs, and other personal and systemic barriers. Our Care Access Navigators and Community Case Manager work with clients to provide information, referrals and ongoing advocacy.  The goal is to partner with clients and empower them to increase their self-sufficiency.
